Each school that operates programs funded through the Consolidated Application must develop a Single School Plan for Pupil Achievement as required by Education Code 64001. The California Department of Education has provided a single, comprehensive school plan template that meets the content requirements of EC 64001. Schools operating School-Based Coordinated Programs or School-Wide Programs may use this template with some additions/adaptations to meet the specific requirements of those programs. The template may also be used as the foundation for II/USP and High Priority Action Plans and CSRD plans with supplemental information as required by those specific programs.
