Forms
This site is designed to give identified members access to the Kern County Consortium SELPA Special Education suggested forms and recommendations for completion of those forms. If you have have questions, call the SELPA Office at 661.636.4801. This site is to facilitate access for designated representatives of the Kern County Consortium SELPA to obtain forms, updates, manuals, handbooks and other materials that are not general distribution items. Each form is listed by the number that has been assigned and a name that has been given to that form. More forms and resources will be added as they are developed in response to your needs and compliance requirements.
Kern SELPA provides many documents as Adobe PDF (Portable Document Format) files, which require the Acrobat Reader for viewing and printing. The benefits of PDF are many, incuding the FREE availability of the Reader, which allows for more compatibility from user to user, regardless of the word processing software installed on each users' computer. Each form may have the district information added to the top of the form before printing by placing the cursor in that area and "clicking" the mouse. The three or four lines of print will be automatically centered. Press <enter> after the completion of the last line of print and the form will be printed with the district information on it.
